Frontal Bossing Plasty in Montreal
The forehead feminization surgery ”Frontal Bossing” is the most commonly requested and most spectacular procedure in Montreal’s gender affirmation facial surgeries.
Understanding Forehead Feminization Surgery:
During puberty, the male skull develops to form a more rounded prominence above the brow line. This area, known as the “bossing” by adipose tissue specialists, extends from the middle of the forehead to the outer edge of the orbits, giving the male face its distinctive feature.
In contrast, females have a skull with a more convex contour. Their forehead appears smoother and flatter with less protrusion. Hormone replacement therapy (HRT) does not affect the shape of the skull, therefore surgical options are considered the most viable solution to refine angle, proportions, and size in order to align with the female form.

The Surgery : Frontal Bossing Plasty
The feminization of the forehead involves an incision from ear to ear, which can be performed in two areas: in front of the hairline or in the area of the scalp where hair is located. During the consultation, Dr. Bensimon can suggest the option that best fits the unique size and shape of the patient’s head.
This is the most requested surgery by patients and the results are impressive and permanent. This procedure is often combined with a brow lift, full smoothing of the orbital crest, reshaping of the orbit, and advancement of the hairline.
Depending on the thickness of the cranial bone, Dr. Bensimon may simply shave the bone. However, in most cases, he must move the bone further back, commonly known as a type 3 forehead. Dr. Bensimon uses an exclusive technique developed over the years that allows for long-term results and preservation of contours through titanium micro mesh. His technique has been published in the famous Aesthetic Plastic Surgery Journal.
These methods are extremely effective for achieving a smooth forehead, and each surgical strategy is tailored to the unique size, shape, and silhouette of the patient. A frontal X-ray or scan is necessary to plan the most effective surgical strategy.
The contour of the orbital edge, brow lift, and lowering of the hairline are also performed at the same time as they can all be done from the same incision.

After the Surgery
- Post-operative care is surprisingly easy as there is very little swelling.
- After a week, visit Dr.Bensimon to have the staples removed from the surgery. The experience is easy, painless, and quick. When staples are in place after surgery, it is recommended to sleep with your head straight on a pillow for 7-10 nights as any slight pressure can cause mild discomfort.
